We’re looking for an experienced Care Manager for a 6-month fixed-term contract to lead and support a dedicated clinical team, ensuring the delivery of high-quality, person-centred care in a welcoming and professional environment.

Key Responsibilities
- Oversee the daily clinical operations of the care home
- Provide leadership, coaching, and support to the clinical team
- Manage performance, training, and staff development
- Ensure compliance with AN-ACC assessments and aged care standards
- Conduct audits and review clinical incidents and feedback
- Lead person-centred care planning and family case conferencing
- Manage medication administration processes and clinical documentation
- Liaise with allied health, GPs, and external care providers
- Current AHPRA registration as a Registered Nurse (essential)
- Post-graduate qualifications in aged care (highly regarded)
- Proven leadership experience in aged care or acute settings (min. 30-bed facility)
- Strong clinical assessment, care planning, and documentation skills
- Exceptional communication and stakeholder management abilities
- Knowledge of the Aged Care Quality Standards and resident rights
