Austcorp are currently seeking passionate and experienced Care Manager for a leading Aged Care provider located in beautiful Geelong.

The facility holds great internal structure, working alongside another Care Manager there is an excellent support and a great team environment.

To be successful you must be a motivated and hands on expert, with excellent leadership and communication skills.

Key Responsibilities

• Ensures delivery of quality clinical and person first care for all residents
• Oversees coordination of ACFI assessments
• To mentor and lead a team
• Overseeing Medication management
• Care planning and evaluation
• Undertake personal and professional development for yourself and your team
• Ensure the facility is consistently meeting the aged care standards

Role Requirements

• Current and valid AHPRA Registration
• Minimum 2 years’ experience in Aged Care as a Clinical Care Manager
• Strong leadership abilities
• Expert clinician and care planning capabilities
• Knowledge of ACFI
• Strong communication and relationship building skills
