About the Role
The Case Manager will:
- Coordinate holistic, resident-focused care including assessments, care planning, service delivery and evaluation.
- Liaise with residents, families, and healthcare providers to ensure care needs are met and independence is supported.
- Access and manage externally funded care options such as Home Care Packages, CHSP, DVA, and ACAT assessments.
- Monitor and guide Care Service Workers to ensure safe, high-quality clinical practice.
- Maintain compliance with Aged Care Quality Standards and support accreditation readiness.
- Promote resident wellbeing by fostering inclusion in meaningful activities and upholding residents’ rights.
- Act as a trusted point of contact for residents and families, ensuring clear communication and effective case management.
About the Candidate
The successful candidate will demonstrate:
- Diploma of Nursing with current AHPRA registration (Enrolled Nurse).
- Minimum 3 years’ experience in aged care, community care, or a similar clinical role.
- Strong knowledge of aged care quality standards, case management, and care planning.
- Excellent communication skills with the ability to engage sensitively with residents and families.
- Ability to build collaborative relationships with colleagues, external providers, and healthcare professionals.
- Solid IT proficiency (Microsoft Office suite) and confidence in documentation and compliance.
- Current CPR/First Aid, valid driver’s licence, and National Criminal History Check (or willingness to obtain).
