Job Title: Clinical Care Manager

A 40-bed residential aged care facility dedicated to providing compassionate and personalized care to our residents. Under new leadership, they are embarking on an exciting journey to rebuild and grow, fostering an environment of excellence and innovation. With a specialisation in homelessness care, we aim to make a meaningful impact on the lives of those in need, offering fulfilling opportunities for dedicated healthcare professionals.

Position Overview: Our client is seeking an experienced Clinical Care Manager to join our team and play a pivotal role in leading our clinical care services. The ideal candidate will possess strong management skills, a passion for delivering high-quality care, and a commitment to serving vulnerable populations. This role presents a unique opportunity to make a difference in the lives of our residents while contributing to the growth and success of our facility.

Responsibilities:
  • Oversee the delivery of clinical care services to residents, ensuring adherence to best practices and regulatory requirements.
  • Develop and implement care plans in collaboration with residents, families, and interdisciplinary team members.
  • Provide leadership and guidance to nursing staff, fostering a culture of excellence, collaboration, and continuous improvement.
  • Monitor and evaluate resident care outcomes, identifying areas for improvement and implementing corrective actions as needed.
  • Act as a liaison between residents, families, staff, and external healthcare providers, promoting open communication and effective coordination of care.
  • Support the facility's homelessness specialty program, advocating for the unique needs of homeless residents and facilitating access to resources and support services.
  • Collaborate with leadership to develop and implement strategic initiatives to enhance the quality of care and resident satisfaction.
Qualification requirements:
  • AHPRA Registration
  • Proven experience in clinical care management, preferably in an aged care or similar setting.
  • Strong leadership and interpersonal skills, with the ability to inspire and motivate a diverse team.
  • Excellent communication and problem-solving abilities, with a compassionate and empathetic approach to resident care.
  • Knowledge of regulatory requirements and best practices in aged care.
  • Experience or interest in working with homeless populations is highly desirable.
