Job Title: Clinical Care Manager

A 40-bed residential aged care facility dedicated to providing compassionate and personalized care to our residents. Position Overview: Our client is seeking an experienced Clinical Care Manager to join our team and play a pivotal role in leading our clinical care services. The ideal candidate will possess strong management skills, a passion for delivering high-quality care, and a commitment to serving vulnerable populations. This role presents a unique opportunity to make a difference in the lives of our residents while contributing to the growth and success of our facility.

Responsibilities:

• Oversee the delivery of clinical care services to residents, ensuring adherence to best practices and regulatory requirements.
• Develop and implement care plans in collaboration with residents, families, and interdisciplinary team members.
• Provide leadership and guidance to nursing staff, fostering a culture of excellence, collaboration, and continuous improvement.
• Monitor and evaluate resident care outcomes, identifying areas for improvement and implementing corrective actions as needed.
• Act as a liaison between residents, families, staff, and external healthcare providers, promoting open communication and effective coordination of care.
• Support the facility's homelessness specialty program, advocating for the unique needs of homeless residents and facilitating access to resources and support services.
• Collaborate with leadership to develop and implement strategic initiatives to enhance the quality of care and resident satisfaction.

Qualification requirements:

• AHPRA Registration
• Proven experience in clinical care management, preferably in an aged care or similar setting.
• Strong leadership and interpersonal skills, with the ability to inspire and motivate a diverse team.
• Excellent communication and problem-solving abilities, with a compassionate and empathetic approach to resident care.
• Knowledge of regulatory requirements and best practices in aged care.
• Experience or interest in working with homeless populations is highly desirable.
