Care Manager – Chinchilla, QLD

A leading aged care provider is seeking an experienced Care Manager to oversee clinical operations and lead high-quality, person-centred care delivery at a well-established residential facility in Mildura.

This is a senior leadership position focused on clinical governance, team leadership, and continuous improvement of care outcomes within a supportive and forward-thinking environment.

The Role

You will play a key role in leading clinical services, supporting operational performance, and ensuring residents receive safe, compliant and high-quality care.

Key responsibilities include:

• Leading day-to-day clinical operations and care delivery
• Supporting and developing nursing and care teams through coaching and performance management
• Driving clinical governance, compliance, and quality standards
• Managing care planning, assessments, and case conferencing with families
• Overseeing medication management and clinical risk processes
• Reviewing incidents, complaints, and implementing improvement initiatives
• Supporting accreditation requirements and regulatory compliance
• Partnering with allied health professionals and external providers

About You

• Bachelor of Nursing with current AHPRA registration (essential)
• Demonstrated leadership experience in aged care, community care, or acute settings
• Strong clinical assessment and care planning capability
• Experience with aged care standards, compliance, and governance frameworks
• Confident team leader with strong communication skills
• Commitment to person-centred care and service excellence
