Clinical Case Manager

An opportunity is available for an experienced Registered Nurse to step into a Clinical Case Manager role supporting residents within a retirement living and community care environment. The position focuses on coordinating integrated care services, ensuring residents receive the appropriate support to maintain independence, wellbeing and quality of life.

This role works closely with residents, families, care teams and external providers to coordinate clinical and community-based services, oversee care planning, and ensure compliance with aged care standards and funding programs.

Key Responsibilities
  • Provide holistic clinical case management for residents, promoting independence and enablement
  • Conduct pre-entry and ongoing assessments to determine care needs and appropriate support services
  • Develop, implement and regularly review comprehensive care plans
  • Coordinate access to Home Care Packages, Commonwealth Home Support Program services and other funding streams such as DVA where applicable
  • Liaise with residents, families and external providers to ensure continuity of care
  • Facilitate case conferences and risk assessments where required
  • Monitor and guide Care Service Workers to ensure services are delivered to the required clinical standards
  • Maintain accurate documentation and ensure compliance with aged care quality standards and regulatory requirements
  • Support resident wellbeing by encouraging participation in meaningful activities and social supports
  • Contribute to quality systems that support compliance with Aged Care Quality and Safety Commission requirements
About You
  • Registered Nurse with current AHPRA registration
  • Minimum 3 years’ experience in a similar clinical or community care role
  • Strong knowledge of community aged care programs including HCP and CHSP
  • Experience coordinating care within retirement living, community or residential aged care settings
  • Ability to build strong relationships with residents, families and multidisciplinary stakeholders
  • Strong documentation and administrative capability
  • Intermediate Microsoft Office skills
  • Current driver’s licence
Qualifications and Requirements
  • Bachelor of Nursing
  • Current CPR and First Aid (or willingness to obtain)
  • National Criminal History Check
  • Ability to travel between sites within the region
