Community Care Manager

Job description

A progressive retirement living provider is seeking a senior community care leader for a 3-month interim contract overseeing Support at Home and fee-for-service care across multiple sites.

This confidential appointment will provide operational continuity, governance oversight and workforce leadership across community-based services within retirement living environments.
 
The Role

You will lead community care operations across independent living and serviced apartment settings, ensuring safe, compliant and commercially sustainable service delivery.
Key responsibilities include:
  • Oversight of Support at Home and community care services
  • Budget accountability and workforce optimisation
  • Strengthening quality, risk and governance frameworks
  • Managing SIRS, complaints and regulatory engagement
  • Supporting aged care reform implementation
  • Driving service utilisation and operational efficiencies
  • Embedding care management systems and reporting processes
  • Leading multidisciplinary teams across multiple locations
  • Partnering with village managers and external providers
Regular travel across sites will be required.
 
About You

You are a commercially aware community care leader who balances resident choice, clinical governance and financial performance.
You will bring:
  • Registered Nurse qualification with current AHPRA registration
  • Significant experience in community and aged care operations
  • Strong knowledge of Support at Home and aged care funding models
  • Proven multi-site leadership capability
  • Experience implementing operational or regulatory change
  • Strong financial and workforce management skills
  • Ability to operate confidently within complex stakeholder environments
